ABSTRACT

In the course of the study, histological preparations of 35 patients aged 16­25 years with incisor crowding who underwent orthodontic treatment with the fixed appliances according to the standard technique, had been analyzed. Three groups of patients had been distinguished: the first one was the reference group, the second one was the group of patients with the fiberotomy conducted before treatment with fixed orthodontic appliances, and the third one was the group of patients with the fiberotomy performed after the removal of fixed orthodontic appliances according to a standard technique used in the Department of Maxillofacial Surgery and Dentistry of Samara State Medical University (SamSMU)

ABSTRACT

Introducción: El propósito de esta revisión sistemática fue evaluar la efectividad de la fibrotomía supracrestal circunferencial como procedimiento coadyuvante en la estabilidad del tratamiento ortodóntico. Métodos: Se realizó una búsqueda electrónica en diferentes bases de datos. Los criterios de inclusión fueron: artículos entre el año 1980-2013, realizados en humanos, estudios que evaluaran la efectividad del procedimiento en un período no menor a 1 año y pacientes que no hubieran sido sometidos a tratamientos periodontales similares. Resultados: Un total de 268 títulos fueron encontrados en bases de datos electrónicas y en la búsqueda manual no se encontraron artículos relacionados con el tema. Un total de 37 abstracts fueron revisados y de éstos se excluyeron 34 por no relacionarse con el tema, finalmente fueron seleccionados tres artículos. La fibrotomía supracrestal circunferencial se considera un procedimiento efectivo para prevenir la recidiva a corto y largo plazo, siendo más efectiva para disminuir la recidiva rotacional que otros tipos de movimiento. El tiempo ideal para realizar la FSC no es conocido y la decisión del clínico es arbitraria y empírica. Conclusiones: Se ha observado una tendencia a la recidiva relacionados con diversos factores como el efecto de la musculatura perioral, la oclusión, el aumento del ancho intercanino, la dirección del crecimiento mandibular, la discrepancia óseo-dental y la influencia de los tejidos periodontales y gingivales. Las conclusiones de esta revisión sistemática deben ser consideradas con cautela debido a que los estudios presentan un nivel de evidencia bajo-moderado. Se requieren ensayos clínicos aleatorizados prospectivos para soportar estos hallazgos.


Introduction: The purpose of this systematic review was to evaluate the effectiveness of circumferential supracrestal fiberotomy as an adjunctive procedure in the stability of orthodontic treatment. Methods: An electronic search was conducted in different databases (MEDLINE, PUBMED, Cochrane Libray). Inclusion criteria was: articles published between 1980-2013,humans trials, studies evaluating the effectiveness of the procedure in a period of 1 year or more, patients who had not undergone similar periodontal treatment between 1980-2013. Results: Of a total of 268 articles, 3 studies were selected for the final analysis. Supracrestal circumferential fiberotomy is considered an effective method for preventing short and long-term relapse, being most effective in reducing rotational relapse than other types of movement. Supracrestal circumferential fiberotomy was found to be an efficient method to maintain the lower anterior teeth aligned in the short and long-term basis comparing with a non treated control group. Conclusions: The tendency to relapse has been related to various factors, such as the effect of the perioral muscles, occlusion, increased intercanine width, mandibular growth direction, osseous-dental discrepancies and the influence of periodontal and gingival tissues. The findings of this systematic review should be considered with caution because the studies have a low to moderate level of evidence. Prospective randomized clinical trials to support these findings are required.
